Insurance Information

We make every effort to provide you with the finest care and help you make informed decisions regarding this care. One way to accomplish this is to work hand-in-hand with you to maximize your insurance reimbursement for covered procedures.  

Insurance Coverage

Please bring all insurance information to the initial visit.  Our staff will help to determine whether insurance will provide benefits for the recommended treatment and what will be your financial out of pocket expense.  Your employer has made a contract with the insurance company that determines what procedures it covers and at what level of coverage. Very few policies cover 100%.

Our staff will do their best to help you maximize benefits.  We can send pretreatment estimates to many dental insurance companies to get a better estimate of benefits. Other companies will only give us an estimated percentage of coverage, or pay on a fee schedule.  These are estimates and not guarantees, based on the situation at the time of service.

Some oral and maxillofacial procedures are covered under medical insurance. We can submit to either/both dental or medical insurance companies.

Surgery Visit as Initial Visit

We have an established required deposit for those patients who have a surgery procedure performed on their initial visit to the office.  We will submit to your insurance company for payment.  If you have over paid your portion a refund check will be issued and if you owe additional money we will bill you for it.  

In some cases when a procedure will be performed on the initial visit we may request your insurance information prior to the visit to allow us to help determine eligibility and benefits.

Please remember that our contract for service is with you and not your insurance company and you are ultimately responsible for the cost of service.
